What Is the Sliding Fee Discount Program?
The Sliding Fee Discount Program provides reduced rates on medical services based on household size and income. Our sliding fee scale helps make care more accessible for those who are uninsured, underinsured, or facing financial challenges—because everyone deserves quality care.
This program supports our commitment to whole-person wellness, ensuring that cost is never a reason to delay care.
Who Is Eligible?
Eligibility for the program is based on:
- Total household income
- Number of people in the household
- Current Federal Poverty Guidelines
Even if you have insurance, you may still qualify for reduced-cost care depending on your financial situation. Our goal is to work with you to make healthcare more manageable and less stressful.

How to Apply
The application process is simple:
- Complete a short application form (available at the front desk of any Meridian HealthCare location or by calling 330-797-0070).
- Provide proof of income—such as recent pay stubs, tax returns, or benefit statements.
- Submit your documentation to our front desk, or call us at 330-797-0070 to have a form sent to you.
- Our team will review your application and let you know what level of discount you qualify for.
